With funding from the Department of Health and Human Services, Health Resources and Services Administration and the Public Health Institute, USAging is providing new tools to help the Aging Network educate their communities on COVID infection rates, safety and vaccinations. SHIP TA Center has developed a new consumer handout explaining Medicare coverage of the COVID-19 vaccine. The handout is available in seven languages and comes with a guide for professionals in English and Spanish titled, Talking about Medicare Coverage of the COVID-19 Vaccine.
 

Hebrew SeniorLife’s R3 Initiative–Right Care, Right Place, Right Time: Effectively Integrating Senior Care and Housing

Hebrew SeniorLife has developed a population health approach to supporting older adults living in congregate housing. A prevention focused, place-based teamworks to develop relationships with residents, connect them to services and facilitate communication between housing staff and healthcare providers.
