Person-Centered Decision Making in Healthcare and at End of Life

This webinar will discuss the benefits and limitations of advance care planning as a person-centered approach to care. Advance care planning and health care directives are written instructions that explain the type of medical care you want in the event that you are not able to communicate your preferences. Speakers will talk about key concepts in healthcare decision making and advance directives and how these intersect with culture, faith, advocacy, lived experience, and community-based services.
