The Aging and Disability Business Institute is hosting a webinar with Partners in Care Foundation. During this webinar, Partners in Care will discuss how their partnership with a local medical provider helped them develop a model of transitional care management and chronic care management focused on addressing the Social Determinants of Health. The webinar will provide an overview of the CMS policies which allow CBOs to contract with medical providers and discuss specifics such as billing codes and reimbursement rates.

The Ventura County Continuum of Care (CoC) has united many public and private care providers under one coordinated entry system. Shelters, state agencies, law enforcement, health care systems, area agencies on aging and other stakeholders all work cooperatively under this no-wrong-door system. This webinar will cover what Ventura County learned in the process and how you can use these lessons to enhance the way you connect at-risk citizens with the help they need. 

The National Center on Law and Elder Rights is hosting a webinar about the challenges of self-neglect and hoarding in older adults. The webinar will review these issues and discuss the root causes. Participants will learn how to distinguish when these problems become legal issues, learn how to provide assistance, and learn what the lawyer's role should be.

The National Institute on the Prosecution of Elder Abuse (NIPEA) is a three day course for elder abuse prosecutors. Participants learn practical skills by working two actual cases. They also explore ethical issues, relevant medical evidence, and the importantce of a victim-centered community response. NIPEA will be held in San Antonio, Texas from November 6-November 8, 2019.
