The Coordinating Council on Access and Mobility (CCAM) is a federal interagency council that coordinates funding and provides expertise on human services transportation for people with disabilities, older adults, and low income individuals. Last week, the council adopted the 2023-2026 CCAM Strategic Plan prioritizing the expansion of safe access to transit and affordable mobility options. The plan also sets a framework for collaboration among CCAM grantees at all levels and across jurisdictions.

This webinar will highlight how two New York-based health care organizations, John R. Oishei Children’s Hospital and The New York Foundling, are engaging patients in new ways to transform their health care delivery practices. Lessons from these efforts can inform health care organizations across the country seeking to involve patients, particularly patients of color, in efforts to advance health equity.

The Jewish Federations of North America (JFNA) Center on Holocaust Survivor Care and Institute on Aging and Trauma is hosting a webinar on end-of-life care for older adults with a history of trauma and their family caregivers. This webinar will focus on how to incorporate person-centered, trauma-informed (PCTI) principles into end-of-life care. CalAIM is California’s new care delivery and payment reform initiative that seeks to expand access to coordinated, whole-person care specifically for people with complex health and social needs.
