Webinar on Opportunities to Enhance Connection with Community Resources

The National Academy for State Health Policy (NASHP) is hosting a webinar about making connections in and out of health care and long-term care systems to acquire services and supports. Community health workers (CHWs) are community members with shared lived experience that can serve as a link between patients and their caregivers to community and social resources. This webinar will discuss state and local programs, trainings, and funding opportunities where CHWs are supporting families and caregivers of older adults.
