Representatives from CHCS and Joslyn Levy & Associates will discuss their measures library, which was created to help assess the impact of complex care interventions within pilot sites participating in CHCS’ Advancing Integrated Models (AIM) initiative. Representatives from two of these sites will share their experiences using the library.
