Annaully, the John A. Hartford Foundation Business Innovation Award recognizes community-based organizations that are working to improve health outcomes and quality of life for older adults and people with disabilities. The runners-up for the 2021 Business Innovation Award were Daybreak’s Rapid Response Case Management Pilot (RRCM) and the Area Agency on Aging & Disabilities of Southwest Washington’s (AAADSW) Community Paramedicine program. These programs worked with multiple community partners to improve the health and quality of life of individuals who are use emergency services.

Building a Continuum of Care for People with Serious Illness: State Strategies to Support Patient Engagement

People who are more engaged in their care are shown to have better outcomes, use fewer resources, and report more satisfaction with the care they receive. This webinar will highlight opportunities to address food insecurity by connecting state partners and centering lived expertise, while discussing recent national exploration done by the Center for Health Care Strategies. It will feature insights from community members with lived expertise as well as state officials from Oregon and Indiana about opportunities to coordinate across Medicaid and SNAP to address food insecurity. Dementia in the Community: Intergenerational Programming Aimed at Engaging Children, Youth and People Living with Dementia

Event Description: 
This NADRC webinar will discuss intergenerational and youth-focused programs that address social and community issues regarding dementia. Research suggests that those living with dementia who participate in intergenerational programs show a decrease in social isolation, as well as an increase in a sense of belonging, self-esteem, and well-being. Programming that targets youth helps communities develop a deeper understanding of dementia and provides support for youth who may be also be caregivers.
