Event Description:
In partnership with nursing homes, hospitals, and community-based organizations, two states have developed innovative programs that have assisted over 5,000 people settle back into homes in the community. In this webinar two panels will discuss the core elements of these programs, the role of community service providers, and how states and local partners can overcome barriers. Participants will also learn about funding sources, tools, and tips for replicating these models in their states.
