Webinar on Community Care Hubs Collaboration with Medicaid Agencies

The Center for Health Care Strategies is hosting a webinar to discuss how community care hubs, organizations that support networks of community-based organizations (CBOs), are helping Medicaid agencies in New York and North Carolina scale and strengthen health-related social needs (HRSN) service delivery and referrals. The discussion will highlight design and implementation considerations for building hubs, based on the experiences of Medicaid agencies in New York and North Carolina.
