Webinar on Age-Friendly Hospital Measure & The Role of Nutrition Care

The John A. Hartford Foundation is hosting a webinar to discuss the implementation of Age-Friendly Hospital quality measure to improve care for patients 65 years and older. The webinar will include an overview of this measure and the role of nutrition care in producing better health outcomes. The discussion on nutrition care will highlight the 4Ms framework, the integration of malnutrition screening, and the role of community partnerships in supporting patients after being discharged from the hospital. The audience will consist of clinical and community-based professionals working to support older adults. Participants will learn about steps to incorporate malnutrition screening into hospital's Age-Friendly efforts and how to build hospital-community connections.
