Health Homes
On November 22, 2013, CMS approved the state’s Health Home State Plan Amendment, with an effective date of July 2, 2013. The Health Home program targets Medicaid recipients with two or more chronic conditions, one chronic condition and the risk of developing another, or one serious and persistent mental health condition.
