Health Care-CBO Partnerships: Improving Health Outcomes & Social Connection

USAging will hold a webinar highlighting winners of the John A. Hartford Foundation Business Innovation Awards. 

The John A. Hartford Foundation Business Innovation Award is given to organizations who engage in innovative partnerships and contracting models that promote social connection and employ creative strategies to achieve meaningful health outcomes. This webinar will feature three organizations and their partners:

• The Agency on Aging of South Central CT collaborates with Yale School of Medicine to provide sustainable, age-friendly health care through a Service Navigator.
• Nevada Senior Services partners with University Medical Center to facilitate dementia-capable care transitions that emphasize social connection after hospitalization.
• Fairfax Area Agency on Aging maintains its 36-year partnership with Inova Health.

These national award winners will share program strategies, present examples and case studies, and answer audience questions.
