Person Centered Planning and Practice Web Meeting

Event Description: 

Person-centered planning and coordination play a key role in the provision of long-term services and supports. The Centers for Medicare and Medicaid Services (CMS) is working with ACL through the National Quality Forum (NQF) to address performance and quality measurement in person-centered planning through several web meetings. This meeting is the fourth meeting of 2019.

Health Coaching and Wellness Planning to Increase Client Engagement in Self-Management: A Conversation with Judith Cook

Event Description: 

Resources for Integrated Care (RIC) is hosting a conversation about promoting self-management for individuals dually eligible for Medicaid and Medicare in wellness planning. The Institute of Medicine defines self-management as “the tasks that individuals must undertake to live well with one or more chronic conditions”. These tasks can include diet, exercise, socializing, and other daily tasks. Judith Cook, PhD- who consults with federal agencies such as the National Institutes of Health and the Veteran’s Administration- will facilitate the conversation.

As part of an ACL supported grant and in partnership with n4a, the USC Leonard Davis School of Gerontology is hosting a webinar providing information on a range of home modifications to support aging in place. Speakers will discuss how design and cost can affect decision-making and consumer choice in order to emphasize the importance of home modification in the accessibility of services for older adults.
