The Centers for Medicare and Medicaid Services (CMS) is working with ACL through the National Quality Forum (NQF) to address performance and quality measurement in person-centered planning through several web meetings. This meeting is the fourth meeting of 2019.
Resources for Integrated Care (RIC) is hosting a conversation about promoting self-management for individuals dually eligible for Medicaid and Medicare in wellness planning. The Institute of Medicine defines self-management as “the tasks that individuals must undertake to live well with one or more chronic conditions”. These tasks can include diet, exercise, socializing, and other daily tasks. Judith Cook, PhD- who consults with federal agencies such as the National Institutes of Health and the Veteran’s Administration- will facilitate the conversation.

Speakers will discuss how design and cost can affect decision-making and consumer choice in order to emphasize the importance of home modification in the accessibility of services for older adults.
ACL is sponsoring a webinar as part of the National Alzheimer’s and Dementia Resource Center webinar series. ACL awards grants in order to help states and other organizations strengthen dementia capable systems to support people living with dementia and those who care for them. This webinar will feature two ACL dementia grantees who have implemented and sustained programs developed through the grants.
Resources for Integrated Care is hosting a conversation that will provide a platform for questions and discussion on how health plans and providers can improve health outcomes for individuals with disabilities who are dually eligible for Medicare and Medicaid. The discussion will also highlight strategies to improve coordination, including the role of interdisciplinary teams. This conversation encourages participation from stakeholders who provide services to dually eligible individuals in order to identify and address gaps in care coordination.
