This webinar will explore policies and practices that payers can use to help providers better address patients’ social needs, such as food and housing access, to improve their overall health. The webinar will include discussion of when targeted payment incentives are effectives and lessons learned by payers who have implemented policies to address social determinants of health.

This webinar will share information about the NCIOA Tribal Long-Term Services and Supports (LTSS) National Resource Center, which seeks to improve LTSS for Native elders. This webinar will provide an overview of the project, help attendees understand the continuum-of-care concept as it relates to LTSS, and discuss webinar trainings and other activities based on the results of a survey of tribal leaders.
