This webinar will discuss the successful cross-sector partnerships of the Eastern Virginia Care Transitions Partnership (EVCTP) and Virginia Area Agencies on Aging Cares (VAAACares). These organizations are bringing community-based organizations (CBOs) together with traditional payers and providers, and are serving as models for achieving the Triple Aim of better care, better outcomes, and lower costs.
Leaders from EVCTP and VAAACares will present on the process that led to Virginia’s success, and share best practices and lessons learned as Virginia state has avoided potential pitfalls and kept the initiative on track. Speakers include, William 'Bill' Massey, president and chief executive officer of Peninsula Agency on Aging; Kathy Vesely-Massey, president and CEO of Bay Aging; and Dr. Kyle Allen, physician and executive will present.
