Webinar on Care Navigators for People Living With Dementia and Their Caregivers

Explore the integral role of Care Navigators in interdisciplinary dementia care teams through insights shared in an upcoming webinar jointly hosted by the Administration for Community Living (ACL), the National Alzheimer's and Dementia Resource Center (NADRC), and RTI International. Care Navigators are valuable members of interdisciplinary dementia care and support teams that assist people living with dementia and their caregivers in connecting with needed resources and providing important insight and information about their clients to the full dementia team. 

This webinar will highlight two programs using Care Navigators: 

  1. CAlz Connect: An ACL-funded initiative through the California Department of Aging (CDA ) that offers dementia care management services to people living with dementia and their caregivers in Imperial, Marin, and Ventura counties, using the Care Ecosystem and HomeMeds evidence-based programs.

  2. OCCK, Inc.: A private 501(c)(3) organization dedicated to helping North Central Kansans with short or long-term disabilities remove barriers to employment, live independently, and fully participate in their communities. OCCK, Inc. is implementing the evidence-based Care Ecosystem in various rural communities with its ACL Alzheimer’s Disease Programs Initiative funding.
