Join the Partnership to Align Social Care for an upcoming discussion with officials from the U.S. Department of Health and Human Services (HHS), as well as key community and health care leaders, on opportunities to address health-related social needs (HRSNs) through community partnerships. This panel discussion will spotlight the Call to Action: Addressing Health-Related Social Needs in Communities Across the Nation released by HHS to catalyze community-level partnerships and call upon leaders in health care, social services, public health, and health information technology to work together to address HRSNs and create a stronger, more integrated health and social care system through shared decision making.
