The Bridge Model is a person-centered, social work-led, interdisciplinary model of transitional care that emphasizes collaboration among hospitals and community based service providers in order to ensure a seamless continuum of health and community care across settings. The Bridge Model National Office is hosting an upcoming webinar, “Doing Your Homework on Hospitals.” It is open to the public and will take place on Tuesday, September 22 from 2:00 – 3:00pm CT (3 – 4pm ET, 12 – 1pm PT).
Sharon Post, Director of the Center for Long-term Care Reform at Health & Medicine Policy Research Group, will lead a discussion about various approaches to learning about hospitals’ performance, financials, and participation in Medicare demonstration projects.
