Bridging Health Care and Social Services for People Living with Dementia and their Caregivers

The National Alzheimer’s and Dementia Resource Center will hold a webinar on how to coordinate care and support services across community-based organizations and healthcare systems to improve health outcomes for people living with dementia (PLWD) and their caregivers. 

In this webinar, panelists will discuss how their organizations integrate healthcare and social services to better support PLWD and their caregivers. Speakers will highlight successful program elements, share challenges faced in implementation and expansion, and offer recommendations for healthcare systems and community-based organizations working to bridge gaps between medical health care and social services.
