Webinar: Connected Care For Older Adults: CHWs & Age-Friendly Care

Connected Care for Older Adults is an innovative model that uses Community Health Workers (CHWs) in partnership with primary care providers (PCPs) to improve care for frail older adults in rural areas. Through Connected Care, CHWs trained in Age-Friendly Health Systems conduct a series of home visits with patients and families and implement Connected Care protocols based on the 4Ms.

The Connected Care for Older Adults webinar is presented in partnership with Oregon Health & Science University and the National Rural Age-Friendly Initiative, a collaborative effort led by the National Rural Health Association with support from The John A. Hartford Foundation dedicated to advancing age-friendly care across rural communities.
