Community Care Hub 101 Learning Series

Event Description: 

The Administration for Community Living (ACL), in collaboration with the Center of Excellence to Align Health and Social Care (powered by USAging’s Aging and Disability Business Institute), is inviting community-based organizations servin gas community care hubs (CCHs) to participate in a four-part Community Care Hub 101 Learning Series. This series, taking place on four Thursdays from mid-November through December, aims to educate and support organizations interested in the community care hub model. These hubs centralize administrative functions and infrastructure to address health-related social needs (HRSNs) and enhance service delivery, with each webinar focusing on foundational elements and featuring examples from leading community care hubs. Registrants will gain access to both remaining webinars and valuable resources to facilitate their engagement in this initiative.
