Event Description:
Annaully, the John A. Hartford Foundation Business Innovation Award recognizes community-based organizations that are working to improve health outcomes and quality of life for older adults and people with disabilities. The runners-up for the 2021 Business Innovation Award were Daybreak’s Rapid Response Case Management Pilot (RRCM) and the Area Agency on Aging & Disabilities of Southwest Washington’s (AAADSW) Community Paramedicine program. These programs worked with multiple community partners to improve the health and quality of life of individuals who are use emergency services. This webinar will teach participants how Daybreak and AAADSW developed their programs and overcame challenges.
