ADvancing States has worked with The John A. Hartford to help improve age-friendly care. The JAHF focuses their work on age-friendly care, dementia care payment, and serious illness care conversations. The report includes accomplishments in these areas including the AARP Public Policy Institute released the 5th edition of the LTSS State Scorecard, CMS announced the GUIDE payment model for comprehensive dementia care, the Center to Advance Palliative Care developed training courses as part of the Age-Friendly Health Care Learning Pathway, and many more.
