Insights from the Accountable Health Communities Model Final Evaluation Report

The Social Interventions Research & Evaluation Network (SIREN), in collaboration with the Camden Coalition, will hold a webinar discussing the Centers for Medicare and Medicaid Services’ Accountable Health Communities (AHC) Model Final Evaluation Report.

The final report details the AHC Model’s impacts on key outcomes, including healthcare utilization, quality of care, service navigation, and net savings. This webinar will include a report overview and a moderated discussion with AHC implementation site leaders from Colorado, New Jersey, and Oklahoma, who will discuss their experiences with model implementation, impacts, and sustainability.
