A Patient-Centered Complex Care Research Agenda: Recommendations for Collective Action

Event Description: 

CHCS has developed the Patient-Centered Complex Care Research Agenda through support from the Patient-Centered Outcomes Research Institute (PCORI). The agenda provides a guide for collaboratively improving complex care with a priority on the experiences of people with complex needs. It was co-created by more than 100 stakeholders, including a Patient and Family Advisory Board. This webinar will highlight the Research Agenda’s recommendations and opportunities for adopting a person-centered approach to complex care research. 

Virtual listening session with health care providers and suppliers on emergency preparedness

Event Description: 

CMS is looking to gather information from health care providers and suppliers on their experiences providing support during public health emergencies (PHE) outside of the COVID-19 PHE. The input and lessons-learned from this session will be used to evaluate CMS regulations regarding emergency preparedness for certain health care providers. CMS is especially interested in hearing input on the following questions:

In 2021, ACL awarded seven grants to support the Community Collaborations for Employment (CCE) Program to help enhance collaborations across systems to maximize experiences for youth with intellectual and developmental disabilities as they transition between school and work in the community. Join the DETAC as three CCE Grantees (Arizona, Kansas, and Massachusetts) share insights about the strategic framework they established to help optimize opportunities for youth with I/DD to achieve competitive integrated employment within tribal reservations, rural, urban, and suburban communities

Building Partnerships to Scale Effective Dementia Care Supports

Event Description: 
Join NADRC for a webinar to discuss Indiana University's (IU) funding under ACL's Alzheimer’s Dementia Programs Initiative to implement the Caregiver Stress Prevention Bundle (CSPB) for people living with dementia and their informal caregivers. The CSPB is delivered through five area agencies on aging (AAA) by trained care coaches who are community health workers employed by the AAAs and supported by IU dementia care experts. Presenters will share how the university partnered with AAAs to implement the dementia care program and results-to-date.

Alternatives to Guardianship in Common Legal Aid Cases

Event Description: 
This session will discuss case studies of common legal aid cases to explore alternatives, ways to ensure effective communications supports, and tools for assessing the client’s mental capacity and their ability to make informed choices. Participants will learn 3-5 less restrictive alternatives to guardianship, “how to” skills and information on how to assist clients in accessing less restrictive alternatives to guardianship, and tools for effective communication with their client(s).
