Webinar: Taking Charge! Evidence-Based Self-Management Programs

The CMS Medicare-Medicaid Coordination Office (MMCO) is hosting a webinar on evidence-based self-management programs.

Managing chronic diseases is often a concern for beneficiaries with both Medicare and Medicaid coverage and the providers and plans that serve them. About half of all adults in the United States have a chronic health condition, such as cardiovascular disease, cancer, arthritis, and diabetes. Chronic diseases are a concern for Medicare-Medicaid beneficiaries because 72 percent of enrollees have three or more conditions, and this is compared to 65 percent of other Medicare enrollees.

This webinar will provide an overview of evidence-based chronic disease self-management education (CDSME) programs. It is important to engage individuals with chronic disease in managing their conditions to ensure the delivery of person-centered care and control the costs and health consequences of poorly controlled chronic illness. This webinar will discuss how CDSME programs can help plans and providers work with patients to take charge of their conditions.
