Supporting Youth with Disabilities Transitioning Out of Foster Care Summit

Event Description: 
Join CMS's Supporting Youth with Disabilities Transitioning Out of Foster Care two-day virtual summit to discuss what challenges state Medicaid agencies and their partners are facing in supporting youth with disabilities transitioning out of foster care. This summit will focus on solutions and propose strategies for state Medicaid agencies and their partners to over come their challenges, by touching on topics of continuity of care, continuity of Medicaid coverage, socio-cultural supports, and employment.
