The National Alzheimer’s and Dementia Resource Center is hosting a webinar about the challenges people living with dementia and their care partners experience when receiving services during and after a hospital stay. During this webinar, participants will learn about how these challenges can be mitigated through well-planned hospital-to-home care transitions. The webinar will highlight Hospital2Home, a care transitions model that helps people living with dementia transition safely back to the community through intensive care coordination.
Presenter: Through his almost 50 year career, Jeffrey Klein has been an innovative visionary health care, aging services executive and consultant. He is currently, President/CEO of Nevada Senior Services Inc. operating adult day care, in-home respite, home modifications, geriatric assessment clinic and wellness programs and is the Aging and Disability Resource Center for southern Nevada. Mr. Klein Chairs the Nevada Commission on Aging Legislative Subcommittee and is an active member of the public policy committees of the American Society on Aging.
