The Chronic Care Act: What it means for you in 2019

The growing population of older adults is creating unprecedented pressure on our health care system and social safety net. States, MCOs, area agencies on aging, health care providers, and community-based organizations must include the implications of the Chronic Care Act (CCA) into their ongoing strategies for serving adults with complex care needs.
