Care Coordination in Managed Long Term Services and Supports

Event Description: 

Older people and adults with disabilities, particularly those with care needs, can benefit from care coordination, and as such, care coordination has been a focal point of managed long term services and supports (LTSS). Care coordinators are typically nurses or social workers who can help with tasks, such as monitoring chronic health conditions, connecting them to social supports, conducting assessments, and writing plans of care. The field faces rapid changes as many states move to expand Medicaid managed care for their populations needing LTSS.

In this webinar, the AARP Public Policy Institute will release a major study by Truven Health Analytics and discuss its findings. Detailed answers on who care coordinators are, what they do, and who they help will be given. Susan Reinhard of AARP Public Policy Institute will moderate, and Paul Saucier and Brian Burwell of Truven Health Analytics will be the speakers.
