21st Century Challenge for Healthy Aging: Balancing Living Well with the Reality of Multiple Chronic Conditions

Event Description: 

HHS leads the Strategic Framework on Multiple Chronic Conditions, a roadmap for improving the health status of persons with multiple chronic conditions (MCC). Since the Framework’s release in December 2010, HHS agencies and external partners have worked together to align their respective programs, activities, and initiatives to advance the Framework’s vision of optimizing health and quality of life for persons with MCC.

This webinar discussion will focus on how HHS and external stakeholders are addressing the challenges for older adults in balancing living well with multiple chronic conditions. Participants will be able to submit questions to the speakers during the webinar.
