The National Council on Aging (NCOA)has released a request for proposals to fund five regional community care hubs (CCHs) to strengthen clinical-community partnerships and rapidly expand older adults’ access to a falls prevention referral pathway that pairs the Centers for Disease Control and Prevention’s STEADI (Stopping Elderly Accidents, Deaths & Injuries) tool with the evidence-based Stepping On falls prevention program.

NCOA is offering five funding awards of up to $717,000 each over 30 months to help CCHs expand falls risk screening, strengthen clinical-community partnerships, and connect more older adults with evidence-based falls prevention programs. Selected organizations will strengthen health care and community partnerships, increase falls risk screening through the CDC’s STEADI tool, and connect older adults to Stepping On, a program that builds strength, balance, and confidence to help prevent falls. The 30-month subaward period is anticipated to run from February 1, 2027, to July 31, 2029.

An informational webinar will be held on Thursday, November 12 at 2:00 p.m.

Interested organizations must complete a form indicating their interest in applying by Friday, November 20. Applications for the funding opportunity are due on Thursday, December 10 by 11:59 p.m.
