Bridging the Gap: Creating Sustainable Dementia Support Through Community Partnerships

The National Alzheimer’s and Dementia Resource Center will hold a webinar exploring how to create sustainable dementia support through community partnerships. 

This webinar will feature from the Kentucky Department for Aging and Independent Living’s Office of Dementia Services. Staff will share how grant funding from the Administration for Community Living supported the launch of an initiative to strengthen dementia-capable services and expand supports for people living with dementia and their care partners across the state. Participants will learn about Kentucky’s statewide implementation strategy and the initiative’s impact, highlighting key successes, lessons learned, and practical strategies that other states and organizations can adapt for their own communities.
