ACL posted a Notice of Funding for chronic disease self-management education (CDSME) programs for older adults and individuals with disabilities. The purpose of this funding opportunity is for applicants to develop a statewide initiative to address the capacity for, and delivery of, evidence-based CDSME and support programs. Additionally, this funding is for the development and disseminate of 508-compliant resources specific to the grant learnings to enhance knowledge in serving the target population and aid in the sustainability of programs. 

Applications must be submitted no later than Wednesday, February 19, 2025 at 11:59 p.m. ET.
