Request for Applications: Community Care Hub National Learning Community

Event Description: 

The application deadline for the 2023-2024 Community Care Hub National Learning Community, overseen by the Administration for Community Living (ACL), is just one week away. Community-based organizations nationwide are encouraged to apply for this opportunity, offering peer support, technical assistance from national experts, and up-to-date information on resources, initiatives, and collaborations aimed at furthering the capacity of community care hubs. The program is scheduled to commence in December and will build on the curriculum and other technical assistance of the 2022-2023 cohorts, including the use of the ECHO Model® “all teach, all learn” approach and individual consulting with subject matter experts. Selected organizations will also participate in a collaborative healthcare learning initiative with the Partnership to Align Social Care for the first four months, focusing on healthcare equity and the use of Medicare Healthcare Common Procedure Coding System (HCPCS) codes. Additionally, ACL will offer a four-part learning series in November and December for emerging community care hubs, in partnership with the Center of Excellence to Align Health and Social Care.

Approximately 30 organizations will be selected, and the program is expected to run for up to nine months. 

Applications are due to ACL by Friday, November 3.
