Building Sustainable CBO and Health System Partnerships under Medicaid Delivery System Reform

The NYC Department for the Aging partners with the NYC Health + Hospitals Performing Provider System, d/b/a OneCity Health Services as part of New York State's Medicaid Delivery System Reform Incentive Payment (DSRIP) program. The program enables better integration of community-based organizations (CBOs) into the healthcare system. One example is the Care Transition Program partnership between NYC Department of Aging’s contracted community-based organizations and two of OneCity Health’s hospitals. During this Aging and Disability Business Institute Series webinar, representatives from the partnership will discuss their successful collaboration.
