Webinar: Integrating Community Pharmacists into Complex Care Management Programs

CHCS is hosting a webinar that will highlight the role of community pharmacies in delivering enhanced services for high-need, high-cost individuals. During this webinar, Community Care of North Carolina will discuss its Community Pharmacy Enhanced Services Network (CPESN) that broadens the capacity for care management and medication optimization services for patients with complex needs. CPESN originated in North Carolina through a Center for Medicare and Medicaid Innovation grant award. This program is now spreading nationally to broaden opportunities for integrating community pharmacists into complex care.
