Fundamentals of the Person-Center Service Planning Process

CMS will hold a webinar on the fundamentals of how to conduct the person-centered service planning process.

The webinar will assist states by:

• Providing an overview of the requirements for the person-centered service planning process and plan as outlined by the 2014 Home and Community-Based Services (HCBS) Final Rule;
• Highlighting promising practices for conducting the person-centered service planning process and developing the plan to accurately reflect what is most important to and for the person; and
• Outlining resources to support states in improving processes for person-centered service planning facilitation and plan documentation.
