The Return on Investment Calculator for Partnerships to Address Social Determinants of Health (ROI Calculator) was designed to help CBOs and their health care partners explore and plan financial arrangements to fund social services for people with complex needs. This webinar will describe how health systems and providers, payers, and CBOs can apply the ROI Calculator to their work. The webinar will feature a Washington State CBO that used the tool to quantify the value of their services when developing a partnership with a Medicare Advantage plan.

 

 

May is Older Americans Month and Community Action Awareness Month. To honor both of these observances, ACL is partnering with ACF's Office of Community Services (OCS) on a virtual site visit to ACL-network provider, North Central Idaho Area Agency on Aging (AAA). The purpose of the virtual visit is to explore the shared goals of the aging network and community services network.

In this webinar, attendees will learn how to develop partnerships for transit planning that is inclusive of older adults and people with disabilities. The webinar will feature speakers from Easterseals Massachusetts and from the Transit Services Division for Frederick County Maryland. Both organizations have partnerships to further transit systems which are inclusive of older aults and people with disabilities. Speakers will share their experiences, best practices, and lessons learned on building inclusive transit partnerships.

This webinar will discuss the development and evolution of the Wisconsin Dementia Care Specialist Program. As part of county-based aging and disability resource centers, the program assists agencies in becoming dementia capable, acts as a catalyst for dementia-friendly communities, and supports individuals living with dementia and their families to live at home in the community.

 

Join this webinar to hear from the National Federation of the Blind (NFB), the nation’s largest and oldest organization of blind Americans with affiliates in all fifty states plus the District of Colombia and Puerto Rico. The NFB offers information, community, programs, and resources to people who are blind and those losing vision, including older adults. Learn about the types of inquiries that individuals reach out for help with; programs and resources through NFB; and examples of solutions for everyday living in the community.

This webinar will help participants understand how to apply the resources shared and will guide the audience in thinking through next steps in implementing suggested strategies.
