The Home and Community-Based Services (HCBS) Conference is ADvancing States’ national conference that annually brings together state, federal, and local policymakers as well as leaders who administer, manage, and deliver waiver and other HCBS programs. The purpose of the HCBS Conference is to share practices and policies that improve state systems delivering long-term services and supports (LTSS) for all ages and abilities.

This webinar is part of a series from CHCS on methods that states can use to strengthen primary care and equitably improve the health of Medicaid participants. This installment focuses on efforts to advance states' health equity goals and improve primary care for communities of color. It will feature presentations about equity efforts at the state and provider levels as well as a question and answer session on how states can support primary care teams through Medicaid managed care and other delivery system and payment reform efforts.
