The Administration for Community Living (ACL) announced a funding opportunity, “Expanding Access to Chronic Disease Self-Management Education for Older Adults with Behavioral Health Conditions.” ACL plans to fund up to five cooperative agreements over a 36-month project period.
The goal of the program is to support the delivery and sustainability of evidence-based chronic disease self-management education (CDSME) programs for older adults with one or more behavioral health conditions. Projects will focus on developing and implementing multi-faceted strategies that expand access to CDSME programs, strengthen referral pathways, address health disparities, and increase availability in underserved communities. Grantees will also collect and report data, share best practices, and support long-term sustainability of programs.
Eligible applicants include state governments, local governments, nonprofits with 501(c)(3) status, institutions of higher education, tribal governments and organizations, and others as listed in the funding announcement. Foreign entities are not eligible. ACL will award up to five cooperative agreements ranging from $1,100,000 to $1,250,000 each.
Applications are due Friday, August 15, 2025, by 11:59 p.m. ET. Letters of intent are not requested.
