Case Management Client Satisfaction Survey Project: Final Report and Recommendations

Medicaid clients and advocates have stressed the importance of case management services to ensure that clients have access to supports, choice among providers and services to meet their needs. Quality became a primary topic of discussion for the Quality for Equality (Q4E) Committee. The Q4E identified concerns regarding the process by which the Department and case management agencies incorporate client input into quality improvement initiatives. Real Choice Community Grant Request for Proposals and Questionnaire

WV Real Choice worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. One way the grant accomplished systems change at a grassroots level is by funding innovative community-based model projects. SPD Consumer Survey: Survey Development and Field Test Report

The Oregon Dept. of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with Human Service Research Institute to develop a consumer survey exploring the satisfaction of people served by SPD in-home Waiver programs (seniors, people with physical and developmental disabilities). The project team worked collaboratively with SPD staff and Stakeholder Group to develop a survey instrument and process that adequately addresses the interests and concerns of all involved.

Participant Experience Training Guide and Survey: West Virginia

West Virginia has chosen to modify the PES E/D to capture both the experiences of participants in the Personal Options waiver (who are able to self-direct a portion of their services), as well as those in the traditional waiver program. WV added an additional domain for Personal Options participants that addresses the adequacy of support staff, resource consultants and financial management services. The survey and training manual are attached.

Long-Term Care Gaps Analysis Survey: Minnesota

Minnesota has shared two of their protocol surveys created as a follow-up to a more extensive, detailed gap analysis completed statewide in 2001. This update has been completed in 2003 and 2005, and will continue to be part of HCBS capacity assessment and HCBS capacity building. The analysis looked at the availability and demand of specific services, including long-term care supports, senior housing and long-term care beds. The 2001 Bulletin is also included for historical information.

Single Point of Entry - Opportunities in Alabama

The Alabama Medicaid Agency contracted with The Lewin Group to conduct a single point of entry feasibility study. Lewin and the Alabama Medicaid Agency staffed a single point of entry work group composed of Alabama consumers and other long term care stakeholders; this work group guided the study. A literature search on SPE best practices and national initiatives was used to develop three possible models for the state, each of which presents an array of pros and cons.

Long-term Care in Tennessee: A Survey of Registered Voters Age 35+

A new survey shows overwhelming support for giving residents choices for long-term care in the home and community. Tennessee is last in the nation in providing options in long-term care. AARP commissioned this survey to determine where voters stand on key issues. Voters say they are more likely to vote for state candidates who support allowing funds now used only for nursing homes to also be available for alternative services which help older and disabled Tennesseans remain independent.
