Elder Choice: How do older people make long-term care choices?

This paper reports the outcome of focus groups, interviews and written surveys conducted in 2003 by a consultant working for the Northern Area Agency on Aging in Wisconsin under a contract with the Department of Health and Family Services. Groups of elders, family members and long-term care professionals were asked to address the question of how, when and why older people make the decisions they do about long-term care.

People Talking to People: Yearly Reports

People Talking to People Building Quality and Making Change Happen accomplishes not just a system change, but also a paradigm shift. Their goal is to create a system of quality assurance and quality improvement that has consumers providing feedback. The feedback gathered during the three year period allows for system wide quality improvement by giving consumers and their family members a voice, thus able to control important choices and decisions in their own lives.

Real Choice Marketing Campaign Awareness Study

These three presentations provide information about the Utah Real Choice marketing and awareness efforts. The “UCare” program was established as part of the grant and the surveys were conducted in 2004, 2005 and 2006. Using telephone surveys, survey participants were asked about their awareness and understanding of the UCare program before and after the marketing campaign.

Measuring Quality Using Experience Surveys & Results

The following two reports are the result of face-to-face and mailing surveys to evaluate user experiences. The face-to-face surveys were conducted with people who are 18 years of age and older who receive Medicaid waiver or Intermediate Care Facilities for Persons with Mental Retardation (ICF/MR) Program Services. The Children/Family Surveys were mailed to families of children under 18 years of age who received services in programs operated by the Department in 2005.

Critical Incident Reporting and Recommendations

One objective of the Texas’s QA/QI grant was to develop a uniform and automated critical incident reporting process. Information was collected on critical incidents reported most often by other states to be used with the Quality Assurance and Improvement (QAI) Data Mart. The QA/QI task force reviewed and used this information to develop a list of critical incident definitions and recommendations.

Critical Incident Reports: Massachusetts

As part of the Massachusetts Independence Plus grant, the workgroup recognized there was a certain amount of risk associated with individualized budgeting. They devised a “Incident Reporting” System that would document any incidents (such as fall, funding problems, injuries) that would compromise the health of an individual with disabilities being served under the program. The forms were created to assist a future Massachusetts IP waiver and have not yet been adopted

Measuring Consumer Outcomes and Satisfaction in California: Identifying a Survey to Provide a Foundation for Quality Management

As part of California’s QA/QI grant, this report discusses grant activity related to the measurement of consumer outcomes. Included in the report are a comparative review of consumer satisfaction survey instruments considered for use and an analysis of California’s information system with respect to the requirements of participation in National Core Indicators.

The Illinois Homeownership Coalition for People with Disability (HCPD) has a comprehensive set of documents pertaining to its advocacy and assistance for homeownership. The organization’s goal is to assist people with disability in purchasing their homes, allowing them to maintain independence and living in their communities.
