Guiding Principles for the Nurse-Consumer Relationship: Improved Collaboration and Support of Consumers\' Community Living

Rutgers Center for State Health Policy convened national and state thought leaders in nursing practice and regulation to develop consensus principles to guide the profession's collaboration with people who want to live in their homes and communities. The 2004 draft principles were revised and endorsed by the American Academy of Nursing's Expert Panel on Aging in November 2005. They are offered as a foundation for further dialogue within and across states.

Paying for Quality Care: State and Local Strategies for Improving Wages and Benefits for Personal Care Assistants

The US is experiencing a severe shortage of qualified direct-care workers to provide personal care services (PCS). Evidence from a number of studies reveals that wages and benefits paid to PCS workers play a fundamental role in determining the quality and quantity of workers. Bridging Silos: Linkages among the DI, SSI, Medicare, and Medicaid Programs, Part I & II

Review two papers that center on the topic of public disability and health benefits and the fundamentals of the Disability Insurance and Supplemental Security Income programs and their relationships with Medicare and Medicaid. Part I looks at definitions, distinctions and overlaps; it reviews the effects on the Ticket to Work and Work Incentives Improvement Act. Consumer and Consultant Experiences In The Florida Consumer Directed Care Program

This report describes the implementation of consumer directed care by synthesizing information from in-person discussions with program staff, a mail survey of program consultants, telephone interviews with consumers in the treatment group, and program records. It discusses the program’s goals and features, the ways beneficiaries managed their program responsibilities and took advantage of increased flexibility, and the degree to which beneficiaries were satisfied with the program.

Centers for Independent Livings (CIL) Pathfinder for Services and Programs for Older Americans

This resource guide was developed to help CILs: 1. understand specific needs and problems that are unique to older persons as they age, 2. understand the network of aging organizations including their organizational structure and funding sources, 3. help identify ways to network with these established “aging” agencies and 4. locate and tap into some of the monies that are available for “senior” programs. The guide has 13 sections covering topics from assistive technology to collaboration ideas.

Housing Information Briefs and Brochures: West Virginia

WV Real Choice worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. The grant worked on many housing activities. Their products include a housing guide, information briefs, brochures, press release and PSA created by the program to explain and address housing needs.

Navigating Youth to Adulthood: A Compilation of Projects and Activities Regarding Youth Transition

Transition can be defined as a continuing process toward independent adulthood. This paper illustrates the time line and products developed to foster improvements in the transition process for students with disabilities as partial fulfillment for the WV Real Choice Systems Change grant. In February 2005, an initial summit was held on youth transition to accurately assess the stance of educators, other professionals and parents. The results of this and future meetings can be found in this report.

Real Choice Systems Change Grant Training Final Report: West Virginia

This grant provided people with knowledge and resources concerning their rights and choices and educated key agencies throughout the state. This report provides an overview of grant activities include the Real Choice Speaker’s Bureau, training sessions and conferences. Trainings were conducted at over 40 Department of Health and Human Resource offices and 20 for the Division of Rehabilitation Services. Three conferences (Transitions, PATHS and People First) were also conducted.

ADRC-TAE: Streamlining Access Self-Assessment with State Examples

How many hoops does a customer have to jump through to receive services? The goal of this self-assessment is to help ADRCs serve as leaders in systems change in the area of streamlined access. The paper outlines several hoops that individuals often have to navigate to gain access to information about LTC services beginning with the initial call. This report contains examples from current grantees, offers context and can be used as a tool for developing a clear strategy for streamlining access.
