Housing Information Briefs and Brochures: West Virginia

WV Real Choice worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. The grant worked on many housing activities. Their products include a housing guide, information briefs, brochures, press release and PSA created by the program to explain and address housing needs.

Navigating Youth to Adulthood: A Compilation of Projects and Activities Regarding Youth Transition

Transition can be defined as a continuing process toward independent adulthood. This paper illustrates the time line and products developed to foster improvements in the transition process for students with disabilities as partial fulfillment for the WV Real Choice Systems Change grant. In February 2005, an initial summit was held on youth transition to accurately assess the stance of educators, other professionals and parents. The results of this and future meetings can be found in this report.

Real Choice Systems Change Grant Training Final Report: West Virginia

This grant provided people with knowledge and resources concerning their rights and choices and educated key agencies throughout the state. This report provides an overview of grant activities include the Real Choice Speaker’s Bureau, training sessions and conferences. Trainings were conducted at over 40 Department of Health and Human Resource offices and 20 for the Division of Rehabilitation Services. Three conferences (Transitions, PATHS and People First) were also conducted.

ADRC-TAE: Streamlining Access Self-Assessment with State Examples

How many hoops does a customer have to jump through to receive services? The goal of this self-assessment is to help ADRCs serve as leaders in systems change in the area of streamlined access. The paper outlines several hoops that individuals often have to navigate to gain access to information about LTC services beginning with the initial call. This report contains examples from current grantees, offers context and can be used as a tool for developing a clear strategy for streamlining access.

CMS Direct Service Workforce Demonstration Grants: Overview and Discussion of Health Coverage Interventions

In 2003 CMS launched the Demonstration to Improve the Direct Service Community Workforce by awarding five grants and five more in 2004. Six grantees are using funds to make health care coverage more affordable/accessible. Interventions vary in strategy but each takes three approaches: subsidizing employer-based coverage, offering arrangements that combine basic insurance and personal accounts, or outreach to enroll DSWs in public private partnerships. This report reviews the lessons learned.

Medicaid at a Turning Point: Results From a 50-State Survey on State Budgets and Medicaid Policy Actions for Fiscal Years 2006 and 2007

The annual 50-state survey of Medicaid officials finds an improved economy combined with the implementation of the new Medicare prescription drug benefit has contributed to the lowest rate of Medicaid spending growth in a decade and the fourth consecutive year in which Medicaid spending growth has slowed. Ohio\'s Quality Framework

What areas of quality are important to individuals with disabilities, families and stakeholders in Ohio? The framework illustrates the areas of quality to be addressed by a functioning quality information management system. Organized by Domains, each section is accompanied by an Outcome. Next, the Core Indicators express how the system will know if the Outcome is present. Performance Measures establish the criteria for measurement and calculation. Rebalancing: Ensuring Greater Access to Home and Community-Based Services

States vary greatly in the proportion of Medicaid long-term care funds expended on home and community-based services. In 2005, Oregon and New Mexico spent over two-thirds of their Medicaid LTC dollars on HCBS while Washington, D.C. and Mississippi spent less than 20 percent on HCBS. To overcome the “imbalance” states, at varying rates, have been “rebalancing” the way that Medicaid LTC services are delivered and financed to allocate a higher proportion of dollars to HCBS.

Ensuring A Qualified Long-Term Care Workforce: From Pre-Employment Screens To On-The-Job Monitoring

In an effort to improve the quality of care provided in long-term care settings, as well as ensure a committed and qualified workforce, ASPE funded a study of the efficacy of various approaches to pre-employment screening and on-the-job monitoring of nurse assistants to prevent resident abuse in nursing homes. This study is intended to inform policymakers, providers and consumers about the contributions and effectiveness of existing federal mandates and state and provider based strategies.

Single Point of Entry - Opportunities in Alabama

The Alabama Medicaid Agency contracted with The Lewin Group to conduct a single point of entry feasibility study. Lewin and the Alabama Medicaid Agency staffed a single point of entry work group composed of Alabama consumers and other long term care stakeholders; this work group guided the study. A literature search on SPE best practices and national initiatives was used to develop three possible models for the state, each of which presents an array of pros and cons.
