Homeownership for People with Disabilities Forms

The Illinois Homeownership Coalition for People with Disability (HCPD) has a comprehensive set of documents pertaining to its advocacy and assistance for homeownership. The organization’s goal is to assist people with disability in purchasing their homes, allowing them to maintain independence and living in their communities. Making Sure the Money Follows the Person in Your State Webcast

ILRU hosted a webcast where Bob Kafka, from ADAPT of Texas and Gwen Gillenwater from the American Association of People with Disabilities (AAPD) offered ideas about how policy planners, agencies, states and advocates can promote Money Follows the Person. There are a number of elements to consider including systemic issues, service coordination, housing, mental health, consumer involvement, quality and accountability. ADRC National Vision and State Models Presentation

The National Association of Area Agencies on Aging (n4a) Conference was held in August 2006 in Chicago. A one day session focused on Aging and Disability Resource Centers. The day was broken into three sections: an AoA/CMS Overview, State Profiles from Ohio, Indiana, Iowa and Illinois and an audience consultation session. Essentials of Housing Teleconference - September 2006

Guest speaker Linda Couch, Deputy Director of the National Low Income Housing Coalition presents federal housing policy updates. Topics include: HUD appropriations for the next fiscal year; proposed changes to public housing and the Section 8 voucher program (expanding the Moving to Work demonstration and almost eliminating public housing authority plans); current information on Low Income Housing Tax Credits, HOME, and CDBG; creating a National Housing Trust Fund, and other timely issues.

Community Options- Development and Implementation of a Continuing Education Seminar

This presentation was developed by the Maryland Department of Health and Mental Hygiene as part of their Real Choice Systems Change Grant. The grant created and executed a Discharge Planning Resource Seminar to educate discharge planning professionals about basic Medicaid services as well as other State-run and privately-funded home and community-based services. The attached presentation reviews the progress, outcome and evaluation of the outreach efforts.

Did You Know, Now You Know training

Provided by Family Voices of Wisconsin, this training is organized into two modules and designed to provide a comprehensive, primary overview of health care and community supports for children and youth. The overall goal of this initiative is to help families access services that will benefit their children. The goal is that one day, they won't hear parents say, \"Why didn't anybody tell me about this before?!\" Presentation slides and evaluations are included.
