In September 2004, Minnesota’s Real Choice Systems Changes grantees presented at the Illinois 2004 Systems Change Seminar. Presentations covered the Minnesota’s Personal Assistance Care Program, QA/QI grant, ADRC grant, and its Senior Health Insurance Program. A variety of topics covered include quality assurance, stakeholder involvement and personal assisted services

The following information was presented at the Illinois 2004 Systems Change Seminar. Topics presented included: trends in long-term care, progress on the Illinois Community-based Treatment Alternatives for Children (CTAC) grants, the Illinois Housing Development Authority and how its work pertains to persons with disabilities, and Michigan’s progress on its Real Choice Systems Change grants.

The Illinois Homeownership Coalition for People with Disability (HCPD) has a comprehensive set of documents pertaining to its advocacy and assistance for homeownership. The organization’s goal is to assist people with disability in purchasing their homes, allowing them to maintain independence and living in their communities. Speaker presentations from the recently concluded 19th Annual State Health Policy Conference in Pittsburgh are now available. Topics include Helping Troubled Youth, High Performance Health Systems, Balancing Long-Term Care, Improving Quality, Assuring Better Child Health and Development, Redesigning Coverage in Medicaid, Patient Safety, and Using Data to Inform Public Policy. ILRU hosted a webcast where Bob Kafka, from ADAPT of Texas and Gwen Gillenwater from the American Association of People with Disabilities (AAPD) offered ideas about how policy planners, agencies, states and advocates can promote Money Follows the Person. There are a number of elements to consider including systemic issues, service coordination, housing, mental health, consumer involvement, quality and accountability. Review presentations from the Chicago National Association of Area Agencies on Aging (n4a) Conference held in August 2006. Presentation titles include: The Maturing of America: Getting America's Communities on Track for the Aging of the Population, On Belonging and Belongings: Older Adults, Katrina and Lessons Learned, Enlightened Leadership: Getting to the Heart of Change.

The National Association of Area Agencies on Aging (n4a) Conference was held in August 2006 in Chicago. A one day session focused on Aging and Disability Resource Centers. The day was broken into three sections: an AoA/CMS Overview, State Profiles from Ohio, Indiana, Iowa and Illinois and an audience consultation session. Guest speaker Linda Couch, Deputy Director of the National Low Income Housing Coalition presents federal housing policy updates. Topics include: HUD appropriations for the next fiscal year; proposed changes to public housing and the Section 8 voucher program (expanding the Moving to Work demonstration and almost eliminating public housing authority plans); current information on Low Income Housing Tax Credits, HOME, and CDBG; creating a National Housing Trust Fund, and other timely issues.

This presentation was developed by the Maryland Department of Health and Mental Hygiene as part of their Real Choice Systems Change Grant. The grant created and executed a Discharge Planning Resource Seminar to educate discharge planning professionals about basic Medicaid services as well as other State-run and privately-funded home and community-based services. The attached presentation reviews the progress, outcome and evaluation of the outreach efforts.

Provided by Family Voices of Wisconsin, this training is organized into two modules and designed to provide a comprehensive, primary overview of health care and community supports for children and youth. The overall goal of this initiative is to help families access services that will benefit their children. The goal is that one day, they won't hear parents say, \"Why didn't anybody tell me about this before?!\" Presentation slides and evaluations are included.
