Person Centered Planning

Michigan developed a six-part training on transition and diversion. Module 4 presents the basic components of a successful transition. Again, person-centered planning is a main component of the transition and helps set the goals for the process, as well as an accurate way to gauge its success. In addition, this module discusses the need for housing, financial management, and transportation in a successful transition.

Michigan developed a six-part training on transition and diversion. Module 3 discusses outreach activities in nursing facility transition and diversion. It reviews different methods of outreach and how they can be developed to be effective forms of marketing. In addition, information for outreach contacts and resources is provided.

Michigan developed a six-part training on transition and diversion. Module 2 is an overview of independent living and person-centered planning. It discusses the history and philosophy of independent living programs and definitions of person-centered planning. This module particularly emphasizes interacting and communicating with persons with disabilities.

Michigan developed a six-part training on transition and diversion. Module 1 is an overview of transition and diversion in the long-term care system. After a brief introduction to nursing facility transition programs, the presentation dispels some myths about the outcomes of transition and diversion programs. In addition, Module 1 discusses different aspects of nursing home transition and the legislation and lawsuits that have affected this process.

These reports are reviews of Utah’s long–term care system and waiver programs and provide recommendations for change. The Focus Group Findings focused on non-institutionalized and community based settings. The Long-Term Care paper provides more generalized findings about the structure of Utah’s long-term care system and how to transition forward based on input from stakeholders and administrators, comparisons to other states, and review of current documents and policy.

MA grantees hired an independent evaluator to conduct qualitative assessments of consumers’ experiences at two and nine months of participation. At two months, the successes included connections to community supports and services. Identified challenges included developing and managing an individual budget. Successes at 9 months include increased independence, self-determination, and availability of workers and supports. Challenges included community liaison training and out-of-pocket spending.

The State of Massachusetts received Real Choice and Independence Plus grants. These presentations describe the projects, their structure, and an overview of grant activities. The goal of the Real Choice grant was to improve the quality and accessibility of home and community-based services available to individuals with disabilities. The goal of the Independence Plus grant was to recreate budgeting so that consumers were able to direct their budget and the services they receive.

This document is from the Aging and Disability Policy Summit. It notes the priority areas regarding aging and disability policy. This worksheet provides a valuable framework for defining issues, activities and goals needed to enact change.

In September 2004, Minnesota’s Real Choice Systems Changes grantees presented at the Illinois 2004 Systems Change Seminar. Presentations covered the Minnesota’s Personal Assistance Care Program, QA/QI grant, ADRC grant, and its Senior Health Insurance Program. A variety of topics covered include quality assurance, stakeholder involvement and personal assisted services

The following information was presented at the Illinois 2004 Systems Change Seminar. Topics presented included: trends in long-term care, progress on the Illinois Community-based Treatment Alternatives for Children (CTAC) grants, the Illinois Housing Development Authority and how its work pertains to persons with disabilities, and Michigan’s progress on its Real Choice Systems Change grants.
