The Aging and Disability Resource Center (ADRC) Demonstration Grant Initiative -Interim Outcomes Report

Forty-three states and territories have received three-year competitive grants since the ADRC program was launched. This report details findings at the overall ADRC level and the pilot level on the outcomes, accomplishments, and contributions of the ADRC program over the grant period, including more immediate results related to key consumer and program outcomes. It also documents lessons learned and program and policy implications at the pilot, state, and national level.

Transitioning Clients with Mental Illness from Colorado Nursing Facilities: A Best Practices Model Report

An advisory committee of consumers, providers, practitioners, advocates and experts worked collaboratively to identify the major challenges preventing people with mental illness from discharging out of nursing facilities and into less restrictive communities. This report is a result of their work and aims to make available a framework for successful transition programs. The best practices described are offered as a guide for service providers, advocates, and mental health clients themselves.

PASS Self-Assessment Tool for Consumer

This tool is designed to equip consumers with the tools necessary to develop a job description, to screen and interview potential employers (personal assistants), and to train new personal assistants as well as address consumers physical needs, leisure and community needs. There are over 30 checklist forms to help determine need. This publication was produced by the Tennessee Personal Assistance Supports and Services (PASS) program.

Checklist for Enhancing the Participation and Input of People with Disabilities

In many ways, consumers are the best experts on their disabling conditions, including their own needs for support and the effectiveness and quality of programs. As a result, obtaining consumer participation and input at all stages of program design, implementation, evaluation and quality assurance is important. Effectively and efficiently involving consumers and obtaining their input, however, requires creative and innovative methods. This checklist will help to enhance full participation.

Medicaid Managed Long Term Care for People with Alzheimer’s Disease and Other Dementias

A growing number of states are turning to managed long-term care in their Medicaid programs for younger adults and elderly people with disabilities, including those with Alzheimer’s disease and other forms of dementia. This issue brief describes how two states, Massachusetts and Wisconsin, have addressed the special needs of beneficiaries with dementia in the design of their programs and makes policy recommendations for states that are considering implementing or modifying programs.
