Person Centered Planning

This guide offers a quick reference on housing resources that can be used to create integrated, affordable, and accessible housing and basic information on how to use these resources. Including community strategies that increase the amount of integrated, affordable, and accessible housing units that usually fall into three general categories: development of housing units; access to existing housing units; and asset development.

Part of the Transitioning to Inclusive Communities (TIC) Project this “how-to” book is for “navigators,” people who want to assist those who wish to move from a nursing facility to the community. The guide gives basic information, plus supportive documents, tools, forms and other resource materials. Sections include: Navigator Roles and Responsibilities, Individualizing Transition/Diversion, the Transition Process, Building a Team, Tools for Success, Responsibilities, Rights and Resources.

This photo essay captures the people behind the design of Oklahoma's Consumer Directed Personal Assistant Supports and Services (CDPASS) design. The Oklahoma Partnership, organized to support Oklahoma's Real Choice System Change and the CDPASS grants. It was also designed to engage Consumer stakeholders in an interactive environment.

The CPASS Hawaii project identified stakeholders, particularly people with developmental disabilities and their families, willing to participate in collaborative efforts towards systems change. This report will elaborate on the accomplishments of the project related to their 5 goals including barriers and areas of success, as well as further recommendations. Also included are two matrixes summarizing accomplishments and training efforts, a demonstration report and an evaluation summary.

These materials help case managers and stakeholders figure out where to get started. The Case Manager Handbook explains the roles and responsibilities, plus worksheets and evaluations. The Self-Advocate Handbook helps identify goals and create a person centered plan. The Community Support Guide and Orientation Presentation provide an overview of terms and ways to implement personal assistance programs. A one page handout on stakeholder values and vision plus the overview brochure are included.

One of two mini-projects aimed at engaging Medicaid-eligible consumers in an experiment with options of self-direction, the purpose of Community Access was to test creative methods that increase the opportunity of children and adults with disabilities to exercise self-determination. The project met the three objectives of supporting at least 12 individuals, develop a community advisor model to identify core competencies, and to identify barriers to full responses for requested services.

This unique and innovative handbook is a guide for educators, providers, support people and/or family members to use as they work closely with youth and adults with significant disabilities, mentoring them as they develop and practice self-advocacy skills that will promote the highest level of independence and success in life activities.

This plan describes fundamental changes that states have implemented and found effective in controlling the public costs of long-term care while insuring that people receive quality services of the type, and in the settings, that they choose.

What do people considering enrolling in a self-directed program need to consider? This presentation describes many of the considerations in detail for those living in Louisiana. The New Opportunities Waiver (NOW) is the service delivery option offered by the Office for Citizens with Developmental Disabilities. The slides offer an overview of key components and responsibilities of self-direction.

This document was developed by the Michigan RCSC grantees as part of their Respite for Children program. The booklet explains the concept of respite, who is eligible for the services, the various models of respite, and how to find the right fit for your family. Next the pamphlet describes how to hire a worker and prepare them for the work they need to perform.
